SUPPORT COORDINATION

1. Support Coordination Level 2Support Coordination

The purpose of the Support Coordinator is to provide support to NDIS participants to navigate the NDIS plan and liaise with service providers in a smooth and effective manner. Support coordinators support you in making the most value of the money provided by NDIS.

Support Co-ordination Empowers People to:

  • Locate the right service provider based on your specific needs.
  • Coordinate home support, other funded support and community activities.
  • Understand what funding is available and how it can be accessed.
  • Provide information about what support services you qualify for.
  • Support you to choose different community activities that you are interested in.

At Arise Services, we aim to work with our participants and their support network to help develop resilience, gain independence, be attentive to their needs and work closely together to resolve any issues there might be regarding their care.

2. Support Coordination Level 3

For NDIS participants whose situations are more complex, they may need a specialist support coordinator. A specialist support coordinator will be allocated to participants who have additional or complex needs related to multiple medical conditions that complicate the nature of the disabilities, will be a qualified and experienced practitioner and meet the conditions as stipulated by NDIS. A specialist Support Coordinator can assist you to manage challenges in your support environment which may include health, education, or justice services and home modifications.

3. Psychosocial Recovery Coaching

The aim of psychosocial recovery coaching is to support you to regulate your emotions, cognition, and social skills that will provide strategies to overcome challenges to deal with mental health, live and work in their communities as independently as possible. Recovery Coaching can help to provide an opportunity to take more control over your life and to assist you to manage any challenges you might encounter in your day-to-day life.
